First of all, I am NOT a networking guru... 
If you're connecting your home PC through an ISP, I think the answer is no. In order for an "IP to IP" connection to work, you need to be able to access the home PC via its IP address which is probably dynamically assigned by the ISP and therefore unknown.
In a typical LAN, each client has an IP address. Knowing what that address is allows you to connect to it to do a network sync. Over the Internet, this is not easily done. I would guess that a VPN (Virtual Private Network) might allow this, but again, I know next to nothing about networking in general.
Perhaps someone more knowledgeable than I (probably 98% of the human population ) can comment further and answer more intelligently.
