Eyemodule fatal alert !

Someone please help me.
I have been having a roblem with my eyemodule of late when I hook it to my Prism. It says
"Fatal Alert
CardSetup.c,Line:521, Card install err"
with the Reset button at the bottom of the alert.
When I click the reset button the same Fatal alert comes on back.
I am so frustrated at this,as my eyemodule was perfectly functional previously.
Please advise me you wizards out there.
