For me, small size was very important in the case as well as having it replace my wallet (the whole point of buying the Visor - datebook/checkregister/calculator/addressbook/games/etc ). One of the biggest reasons I chose the visor was that it was smaller than a lot of the competition. The only serious size competition it had was the Palm V and the expension slot and much lower price finally got me over to Visor's camp (plus the 30 day trial period - day 28 and counting and loving it! )
Part of my issue is I don't wear a belt to hook it on too and I have only a maximum of two pockets typically. So, space is at a premium.
Additionally I wanted it to be syncable in the case (impossible with most cases), highly protective of the visor (especially the screen and buttons), allow for a larger possible future Springboard (e.g., MiniJam), and have space for a real pen, spare batteries, money, checks & deposit slips, credit cards, drivers license, etc. Hmm... I need to fit a larger volume into a smaller sized space. Looks like I might need transdimensional engineering here... Of course though, you've heard of origami and tesseracts! 
Lastly, NOTHING could interfere with usablility of the Visor! Oooo... The crowd is on the edge of their seats, the suspense builds 
I looked at a lot of the cases out there and virtually all of them are too big for my tastes and rarely elegant enough to boot.
Sooo... I designed and made my own. I am very pleased with it. It is a three fold design, uses a leather like fabric over aluminum. It carries everything I wanted and measures just 2.1cm x 12cm x 8.8cm at max when filled with money, credit cards, checks, 2 extra batteries, a top quality real pen, etc, and the Visor of course. That is actually smaller than the Visor + cover in the case that came with my Visor Deluxe package (3.5cm x 13.3cm x 8.3cm). Neither has a belt clip, but either could have one added. In fact, now that I look at it, because of the three fold design in mine it has a built in belt clip, I just never use it or thought of it that way... Too bad I don't have a belt! 
So, if you don't see what you like out there, try making your own and you may come up with a better design that fits your own needs to a Tee.
