I've got the first generation CF adapter, in which the cf card sticks out. I'm pretty happy with it, but it's by no means the ideal solution. I think of it as the library I keep around in case I need it, and accept that it takes a while to get a book.

Right. Forgot about that. I can't speak to the battery drain on anything else but the CF module. I'd say it shaves 1-2 days off of my NICD rechargeables. I can't speak to the other solutions.
Basically, here's my advice for you:
Figure out how much you can afford to spend & just how much memory you think you'll need in the expansion slot
If the amount of memory is over, say, 10 megs, go for either the CF or the SM (me, I like the CF, but the differences bewt. them are pretty much personal pref. & a matter of your desire to wait. With the CF, you wait for software solutions, with the SM, you wait for the module!).
If it's under 10, get the Hagiwara. If you're really expecting to be a power user, cramming all kinds of maps, web pages, & so on, get the internal upgrade.
If cost is an issue, get what you can afford!... but since you're a physician, I'll asume that's not the case
Finally, have you looked at the modules at PalmGear. There are medical references there. I don't have a clue as to whether they're good or not. Maybe that's all you need.
(p.s. This is just me: on my 8 meg VDX, I keep Avantgo & a bunch of web pages, Anna Karenina full text, a pile of games, WordSmith, Quicksheet, a big old pile of personal databases (CDs, Book lists, etc), and more, and still manage to hover around 700k free. On my CF card I store extra books, back up my apps, store extra games, maps, etc, and switch back and forth as necessary.)
