Although this may well be something you have already tried, the one time I wasn't able to hot sync, it turned out that my computer had somehow lost the USB driver. When I unplugged the USB cradle, restarted the computer, and plugged it in again, the computer recognized "new hardware," and all was well. Hope that offers some inspiration.

You didn't specify the "easy stuff" you tried, so maybe you've already tried this, but have you rebooted the computer, and reset the Visor?

1-dust on the craddle contacts, or on the device side (you hold it in your hand without the cover in the back? then you might be putting your hand grease on the contacts)

2-usb cord misconnect (though! keep your feet away!)

3-you though you would select some mode in the hotsync software (tray icon menu) but the checkmark IS IN FACT A TOGGLE. Check again and the checkmark will toggle again. Ensure it is enabled. I had this surprised a few time befor I got it. I was reinstalling each time...

4-your playing you mp3 so loud the the megabass from your megasupersubwoofer is shaking the craddle, causing sparks that interfere...!

If other tacks fail try this...

The two most recent cases in which I have lost the ability to hotsynch, both computers were infected with a virus. Check to make sure you haven't picked something nasty up from the information supergarbageway.

Phase two strategy would be to uninstall the Hotsynch manager in the add/remove programs section of the control panel. Reinstall from scratch. That should remedy any blown conduit files.

Also check for powdered sugar on the Visors's synch pick-ups.

something different

Heres one that worked for me... on the visor go into Hotsync.. From the menu select Connection Setup.. Setup a new connection (Custom) and select it to be USB/Serial to PC or just USB... Then have the new one (Custom) select in the main HotSync screen.. this got my USB going again after 6 months of using a serial cradle!!

Note that this only works in OS 3.5 and above so it's not for anyone using a 2mb Visor or Deluxe.

Lightbulb After a reset

I've noticed that when I install new software to my Prism and the installation requires a reset, the next hotsynch doesn't work unless I unplug the USB and then plug it back in so the OS re-detects the cradle. No PC reboot required. I'm using XP Home.
