Visor lanyard
Does anyone know of a simple lanyard (neck strap!) for a Visor that clips directly to a Visor rather than via a case?

Not commercially. I would think the problem is where to attach the lanyard to the Visor's case (Springboard slot isn't trustworthy, and unless you attach it with 2-sided tape or super glue, the case plastic is too thin and the components are too close to put screws into). Besides, the one time you lean over to wash your hands and submerse your Visor...
You could bug these people to make one.

I wonder if you could mold a heavy rubber bumper with a couple eyelets for a lanyard?
On this topic- I have been playing with what is turning into a pretty useful idea. I noticed the slipcase that came with the Visor has openings on the bottom sides, so I ran a nylon cord through it, ran it through a piece of tubular nylon webbing (for padding), and tied a Fisherman's Knot in it. (This is a knot that allows some adjustability by sliding back and forth. It is pretty easy to tie, but awkward to describe- there are some good sites that show it if you search for them.)
Result- a neck case that holds the Visor pretty dang well with a FlipCover, Flip Lid, or original snap cover on it. The Visor slides out the bottom in a smooth, natural move, and the whole rig is light, attractive (depending on the cordage and padding), and rides under a shirt nicely.
I am a bit paranoid about the Visor falling out, so unless I carry it under my tucked-in shirt, I slip a heavy rubber band around it. I will probably add a Velcro or snap strap for extra security (even though it works great without any such protection, I suspect that the case will loosen up over time.)

I was going to do this also. I like the way the Gameboy uses a metal stud for a lanyard attachment and I don't know why all these devices don't have it. Even my pager has one.
Anyway I thought that a small metal ring could be put over one of the screw studs in the top of the visor. Just remove the front plate and remove a little plastic on each side of the stud. Leave as much plastic on the sides of the stud as possible to fit your metal ring. Then reassemble with the ring in and you will have a fairly strong place to mount a lanyard.
