Amazon Rebate Question

I got a Treo 600 from Amazon earlier this month, it's now activated by Sprint but I have some questions about the rebate:

- The rebate form says to submit "a copy of your first month's service bill with activation"

My Activation was done by a Sprint Store, I paid the 36.00 activation fee then and have a paper contract. Do I submit that or do I need to wait until January when I receive the Monthly Invoice?

- The Sprint Store Rep wouldn't give me a copy of the Sprint Rebate Form (he said it only applied if I bought my Treo there). Will the printed out one from Amazon work despite that they say they wont accept copies ?
