$250 Digital Link Credit
Called today and was shocked to get a rep who knew exactly what I was talking about. He did the credit without any fuss. FYI, I have not used the phone in months (I've actually been through two phones since then) and it doesn't matter.
Now I am just waiting to see if the "equipment credit" will simply appear on my account as a regular credit (as most of Sprint's equipment credits do) or if it really can only be used toward a new phone (which is pointless since I already bought a replacement). Here's hoping I can use it toward service.

Follow up
The credit appeared on my account today. Appears to be a regular account credit which means it can be used for service, not just new equipment (which is only fair since I just bought the Treo anyway!)
Do you just call and ask for a credit, or do you have to send the phone in?
All you do it call. They see that you bought it at some time before they started giving them away and they credit you. Even if you no longer own the phone you get the credit. Too bad I waited a week too long to get it on eBay for about $180 before they started giving them away (now you can't sell them for anything)!
(I think the spare battery and the headset are teh most valuable parts now...say for a $250 purchase only 8 months old!)
